درسی&raquo; و 2سری فتوکپی آن\r\n\r\nتبصره:پذیرفته شدگانی &nbsp;(ورودی مهر وبهمن )که تا 31\/06\/96 موفق به گذراندن کلیه واحد های درسی خود نشده اند حق ثبت نام نخواهند داشت، لازم به ذکر است &nbsp;در صورت ابلاغ و دستور العمل ثبت نام از سازمان سنجش وآموزش کشور قبولی آنها محفوظ خواهند ماند بدیهی است این افراد در صورت فارغ التحصیلی و اخذ دیپلم تا پایان دی ماه 96 می توانند در بهمن ماه ثبت نام وادامه تحصیل دهند.\r\n\r\n5- داوطلبان لازم است مدرکی که وضعیت نظام وظیفه ایشان را باتوجه به بند 3-3 مندرج در صفحات 2-الی4.&nbsp; دفترچه راهنمای آزمون کاردانی پیوسته نظام جدید سال 1396\r\n\r\n&nbsp; &nbsp; &nbsp; &nbsp;&nbsp;6- در صورت داشتن پایان خدمت یا معافیت دائم 2 سری فتوکپی آن.\r\n\r\n7- عکس 4*3 پشت نویسی شده 6 قطعه.&laquo; تمام رخ پشت سفید مطابق با شئونات دانشجویی&raquo;\r\n\r\n8- اصل فیش پستی درخواست تاییدیه تحصیلی دیپلم و 1 سری فتوکپی آن.( این فیش را از دفاتر پیشخوان تهیه نمایید. )\r\n\r\n&nbsp; &nbsp; &nbsp; &nbsp;9 &ndash;.داوطلبان می بایست فرم های ثبت نامی موجود در سایت دانشکده از شماره 1 الی 7 را که در پیوست قرار دارد دانلود و با دقت وبدون خط خوردگی با خط خوانا تکمیل و در موقع ثبت نام به کارشناس مربوطه تحویل نمایید.صحت اطلاعات درج شده در فرم های ثبت نام به عهده پذیرفته شدگان بوده و کلیه
